Liverpool Hospital is one of NSW’s busiest public hospitals and a major discharge point for participants acquiring disability support needs. Prime 1 Care has experience in rapid-onset support setup and works directly with hospital social workers and discharge planners to get participants home safely.

To access the NDIS, you need to be under 65, an Australian citizen or permanent resident, and have a permanent and significant disability that affects your daily activities. Liverpool residents can contact the NDIS on 1800 800 110 or speak to their GP or local area coordinator. Prime 1 Care can help you understand the eligibility process and prepare your access request.
Absolutely. Our support coordinators in Liverpool regularly assist participants with plan reviews gathering evidence, writing participant statements and attending review meetings. If your current plan underfunds your needs, we will help you build the case for a higher allocation. This is part of our support coordination service.
Yes. While our primary service area is the Liverpool LGA, we regularly support participants in the adjoining Fairfield LGA including Cabramatta, Canley Vale, Fairfield, Wetherill Park and Bossley Park. These communities are adjacent to Liverpool and many participants have family and social connections that cross the LGA boundary.
Our Liverpool-area staff include workers with Arabic, Vietnamese, Khmer, Assyrian and Pacific Islander language capabilities. We can also arrange NAATI-accredited interpreters for formal NDIS planning meetings and assessments. Please let us know your language preference when you first contact us.
