Complex disability support

When physical, psychosocial, behavioural or living needs overlap, support planning needs a clear picture of the person and the people involved.

A whole-person conversation

Start with what the participant wants their life to look like, the support already in place and what needs to change. Complex care can bring together personal support, community nursing, supported living and coordination with existing clinicians.

Match the plan to the level of need

Some enquiries involve acquired brain injury, neurological conditions, complex physical disability or psychosocial disability. A diagnosis alone does not determine suitability. We need to understand the tasks, staffing, environment and clinical input required.

Before support is agreed

A complex referral should clarify consent and decision-making, current care and behaviour support plans, worker competency, equipment, medication responsibilities and escalation arrangements. Ask the team to confirm exactly what can be provided in your region.

High risk and changing situations

Discuss foreseeable risks and how the participant’s rights and preferences will be protected. Where a referral involves hospital discharge or a justice interface, the relevant services need to agree their responsibilities. Urgent timing does not remove the need for a safe transition.

Which participants may be appropriate for complex support?

Suitability depends on the specific assistance required, the available workforce, clinical oversight and the proposed setting. Prime 1 Care can discuss complex physical and psychosocial needs and review whether an appropriate support arrangement can be made.

Can complex support include 24 hour assistance?

Around-the-clock support may be discussed for an appropriate supported living arrangement. Awake overnight, sleepover and on-call arrangements are different. Hours, staffing and escalation processes must be agreed before services begin.
