Support involving specialised daily personal activities needs the right care plan, worker skills and clinical oversight for the individual.

Begin with the specific task

Explain the assistance required rather than relying on a general label such as complex care. Prime 1 Care’s community nursing information includes enteral feeding support, catheter and continence care, wound care and medication support. Confirm the exact task, delivery model and local availability with the team.

Nursing and support worker roles

Some tasks require a registered nurse. Other activities may be undertaken by appropriately trained support workers within a participant-specific plan and applicable requirements. The team must clarify who is responsible and how competency is established for the proposed support.

Care planning before the first visit

Discuss current clinical instructions, equipment, infection prevention, communication needs and what to do if the participant’s condition changes. Support should not start until the required plan, skills and escalation arrangements are in place.

Changes and escalation

Ask who reviews the care plan, how incidents or changes are reported and which clinical service is contacted when needed. General enquiries are not monitored as an emergency clinical line.

Who provides high intensity disability supports?

Prime 1 Care welcomes high intensity support enquiries through its disability support and community nursing services. Availability for a particular task depends on assessment, appropriately qualified or trained workers, the care plan and the location.

Can I book a high intensity service immediately?

A referral is the beginning of an assessment conversation. The team must confirm the task, plan, required competence and workforce availability before agreeing to a start date.
