Discuss personal care, transfers, positioning, meals, communication and participation in the community. Support should reflect what the person can do, their preferences and how they want assistance to be provided.
Acquired brain injury and neurological disability
An acquired brain injury or neurological condition may affect physical function, fatigue, communication, memory or behaviour in different combinations. Planning should consider these individual effects alongside existing clinical and therapy advice.
Equipment and the home environment
Discuss access, mobility equipment and any transfer or manual handling instructions. The responsibilities of support workers, nurses, therapists and equipment providers need to be clear before support begins.
Care as needs change
Changes after a hospital admission or over time can affect support hours and worker requirements. Ask how a change in needs will trigger review of the agreed support plan and arrangements.
