Raising the Standard of CareNational Enquiries: 02 9129 0300

National referrals

Refer a participant.

A clear national pathway for participants, families and professionals, including enquiries about complex care and planned transitions.

Start with a conversation.

  1. Tell us the essentials

Your contact details, the service region and the broad support required are enough for an initial enquiry.

  2. Discuss suitability

The team clarifies needs, authority to share information, service capacity and the information required next.

  3. Agree the next steps

Before support starts, confirm the plan, workers, funding or payment, responsibilities and start date.

National Enquiries

We welcome enquiries from participants, families, support coordinators, recovery coaches, hospitals, health and allied health professionals, guardians, case managers, NDIA representatives, justice services and community organisations.

For a hospital discharge or other time-sensitive transition, call first to discuss capacity. We do not guarantee an immediate placement or response time.

A referral is an enquiry. It is not acceptance of a participant, approval of funding or a confirmed service arrangement.

For time-sensitive enquiries, call 02 9129 0300. An urgent enquiry does not confirm availability.

Do not include a participant’s name, NDIS number, diagnosis, offence history, clinical reports or other sensitive records. The team can discuss secure information sharing after the initial suitability conversation.
